Mill Creek Psychiatry strives to help you to make the best use of your medical and mental health benefits. We are happy to provide you with an estimate of your costs and the portion insurance is likely to cover. Since insurance plans are complex and varied, it will be an estimate only. The actual cost may vary.

Cash Pay / Not Using Insurance

For a variety of reasons (such as an increased degree of privacy) some people choose not to submit claims to their insurance company and choose to pay for the sessions out of pocket instead.  You have the right to receive a “Good Faith Estimate” explaining how much your care might cost. No Insurance

Unfortunately, we are unable to offer a sliding scale at this time.

First Choice Health Network, Premera & Regence

Dr. Hajek is a contracted psychiatrist for First Choice Health Network, Premera, and Regence.

If you belong to these plans, as a service to you, Mill Creek Psychiatry will bill the insurance company on your behalf. You just pay the estimated copay or co-insurance (and/or deductible if applicable) at the beginning of each visit.

Since the mental health benefits vary for each plan, Dr. Hajek strongly recommends that you call your insurance company prior to your visit. Below, we have provided a helpful list of questions to ask your insurance company so you know exactly what your benefits will cover and what charges you will be responsible for.

The more that you know about your coverage, the better you can plan ahead and participate in your treatment decisions.

Blue Cross & Blue Shield – Most Plans

Blue Cross and Blue Shield Plans are located out of state (usually where the insurance holder’s company headquarters are).

As a result, BCBS plans contract with a local Washington state insurance company (typically either Premera or Regence) so that you, as a BCBS member, can have access to health care providers in your area.  Since Dr. Hajek is a contracted provider for both Premera and Regence, it means that your visits are likely to be covered under most-but not all-BCBS plans.

If you belong to these plans, as a service to you, Mill Creek Psychiatry will bill the insurance company on your behalf. You just pay the estimated co-pay or co-insurance (and/or deductible if applicable) at the beginning of each visit.

Other Health Insurance Plans

If you are not a subscriber of the above health plans, you can potentially still see Dr. Hajek as an “out of network” provider. Seeing an “out of network” psychiatrist means that your insurance often pays for part of the visit and that you would pay the remaining balance.

You will be asked to pay the full visit fee out of pocket at the beginning of the visit.  Dr. Hajek can provide you with the information for you to submit a claim to your insurance company for potential reimubursement if you have out of network benefits.
